Azithromycin reduces airway neutrophilia and interleukin-8 in patients with bronchiolitis obliterans syndrome

Summary

Fourteen lung transplant patients with BOS (between stages BOS 0-p and BOS 3) were given azithromycin in addition to their existing immunosuppression, and bronchoalveolar lavage was performed before and three months after the start. According to the abstract, FEV1 rose from a mean of 2.36 litres to 2.67 litres and the percentage of lavage neutrophils fell from 35.1 to 5.7 percent. Six patients were responders (FEV1 increase above 10 percent) and eight were not. Responders had higher initial lavage neutrophils and interleukin-8 and had started treatment earlier after transplantation. The authors conclude that azithromycin reduces airway neutrophilia and interleukin-8 messenger RNA in BOS, and that lavage neutrophilia may predict the FEV1 response.
