Intra-subject variability in oscillometry correlates with acute rejection and CLAD post-lung transplant

Summary

Of 289 bilateral lung recipients enrolled for oscillometry before spirometry (December 2017 to March 2020), 37 developed CLAD and 29 had oscillometry at onset; they were time-matched with 129 CLAD-free recipients. The A-score of cumulative rejection was positively associated with variance in oscillometry. Higher variance in measures of ventilatory inhomogeneity (X5, AX, R5-19) was independently associated with higher CLAD risk (p below 0.05), with no association for variance in percent predicted FEV1. The authors suggest oscillometry monitoring could permit earlier identification of graft injury. The number of CLAD events is small. The journal is Frontiers in Medicine according to Crossref.
