Summary
Explant tissue from 52 clinically defined BOS patients and 26 non-CLAD patients (1993 to 2018) was analysed for airway lesions, vasculopathy and fibrosis. Bronchiolitis obliterans lesions were present in 38 BOS patients (73 percent) and varied from concentric fibrotic to inflammatory; 10 of 14 BOS patients without them showed vanishing airways. Chronic vascular abnormalities were found in 22 (42 percent), and Ashcroft score of 5 or above fibrosis in 28 (54 percent). Non-CLAD patients had no obliterative lesions and lower fibrosis scores (p=0.0038). The authors conclude BOS explants show varied histologic findings, including vasculopathy and fibrosis. Explants represent end-stage disease.
