Summary

This investigator-initiated, double-blind, placebo-controlled phase II trial in nine European lung transplant centres randomised 90 adults with bilateral transplants and progressive BOS (477 screened; 48 to pirfenidone 2403 mg per day, 42 to placebo) for 26 weeks. Both groups continued to decline in FEV1 with no significant difference in intention-to-treat, imputed or per-protocol analyses, and graft loss, death and retransplantation were similar. Treatment-related serious adverse events were evenly distributed. The authors conclude pirfenidone did not show superiority and cannot be recommended for BOS.
