Ventilatory capacity in CLAD is driven by dysfunctional airway structure

Summary
The authors applied ex vivo whole-lung and tissue-core micro-CT to explants from six controls, six BOS, three RAS and three mixed CLAD lungs (2014 to 2021, Leuven, Belgium). BOS and mixed explants showed obstruction of proximal bronchioles from generation five, while RAS had obstruction in the most distal bronchioles (generation above 12); 76 and 84 percent of bronchioles were obstructed in BOS and mixed, against 22 percent in RAS. Obstruction was mainly lymphocytic inflammation or constrictive bronchiolitis, and proximal bronchiolectasis and uneven distal ventilation were present in all phenotypes.
